I've implemented this functionality for myself, if you consider it is worth adding to WinMerge the patch is in the attachment. The patch is against trunk rev.5590 (HEAD for the moment).
For now Ctrl+Right Click shows Shell Context Menu for left files and Shift+Right Click shows it for right files. If selected files belong to different parent folders Context Menu is not shown.

Items with patches should not be submitted to the feature requests -list as the danger is they get lost in the list. As there is no easy way to find items with attachments in sf.net. :( Patches-list is the correct place. I'll comment the patch in the patch item you submitted:

It's my fault that I've submitted the patch to the wrong place, should have read documentation carefully.
Anyway, today's patch is much more complete ans usable, maybe it's good that you've missed this old one :)
I'm closing this request item now that the patch has been committed to source control. And will be a part of the next experimental release.
